REFEREE REQUIRED FORMS

All referees, regardless of sport, must complete the following W9 tax form and return.

  • »  W9 Form
  • »  Proof of age (birth certificate, drivers license, passport, etc.)
  • »  Independent Contractor/Waiver Form
    • »  Sign-in to our referee registration area
    • »  Enter your user name (first name + 5-digit NSC Referee ID#, ex. Sam09876) and password (sport you officiate + 5-digit NSC Referee ID#, ex. hockey09876)
    • »  Update your personal information and click submit
    • »  Click the Independent Contractor Waiver link on the left hand side
    • »  You will be presented with an electronic form that requires a box to be checked and then click submit (if you are not presented with the electronic form you have already submitted it). This completes the process.
    • »  Tips:You must add a preceding zero to your 4-digit NSC Referee ID# to make it 5 digits. This page cannot be accessed from behind a firewall. If you have issues accessing this site from a work computer, try using your home computer. If you can log in, but have issues navigating past the initial personal information page (you are logged out or receive an error message), try changing your “cookies” settings. In Internet Explorer, from your ‘Tools’ menu, select ‘Internet Options’. Click on the Privacy Tab. Change your setting to ‘Medium’ and try the page again.
  • »  Current Officiating License
    • »  Soccer: Current USSF Referee Registration (NON-MINNESOTA REFEREES ONLY - MN referees must be regsitered with MN State Referee Committee)
    • »  Hockey: Current USA Hockey registration
    • »  Football: Current Officials Registration from your Association
    • »  Lacrosse: Current US Lacrosse Referee Registration
